Have you told Exchange that its okay for your webserver to talk to it? (by going to Server Configuration -> Hub Transport)
Have you gone into Exchange Management Console and looked at the user -> Mailbox Features and made sure that IMAP4 is enabled?
Are you seeing anything in the error logs (PHP, or Event Viewer)?
